Complete the contact form to make your booking. Please check which tier option you are looking for. Please note that tier 2 and 3 include a case history intake form that will need to be returned no less than 24hours before your scheduled appointment.

For tier 1 you can put your three questions directly into the message box with any relevant information. Email responses for tier 1 will be sent upon payment being received.

After submitting you will be contacted with payment details and confirmation of your booking date and time.
